Responsible for providing hospice social work services and counseling to patients and family units served by Orange City Area Home Health & Hospice. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct case work services, bereavement services, and community education, outreach and referral. He/she also coordinates the Hospice volunteers and the day to day provision of volunteer care. The hospice social worker is an integral part of the Hospice interdisciplinary team (IDT). Number of Hours: Part- Time 24 hours/week Schedule: 3 days a week Educational Requirements: Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and one year of experience in healthcare. Other: He/she must be able to see patients and families within a 30 mile radius of Orange City.
The RN delivers patient care through the nursing process of assessment, planning, implementation and evaluation. He/she is required to carry out all general staff duties in acute care. The Prairie Ridge Care Center (PRCC) Host/Hostess is largely responsible for setting the welcoming, home like atmosphere in the cottage to which she/he is assigned. She/he sets tables, greets residents coming to the dining room, serves the meals, clears tables, runs the dish machine in the cottage, attends to assigned cleaning tasks, does scheduled activities with residents, serves snack times for residents, staff and visitors, and in general watches over and spends time with residents making the cottage a home. Number of Hours: Casual: 0-15 hours/week. Schedule: No set schedule, pick up shifts as available. Educational Requirements: High school diploma desired. He/she must take a modified diet and sanitation course provided through the dietary department and a feeding class provided by our nursing department. Join a long-term care facility where the overriding concept is that PRCC is a HOME – and that means that the care and activities are resident-centered. The Certified Medication Assistant (CMA) performs a variety of resident care activities (medication administration, activities of daily living (ADL’s), documentation, etc.) and related health care functions under the direct supervision of an RN or LPN. Number of Hours: Full-Time: 32 – 40 hours/week; or Part-Time: 15 – 31 hours/week, or Casual: 0-14 hours/week. Schedule: 12 hour-shifts. 6pm – 6am. Positions include every third weekend and holiday. Educational Requirements: Completion of a 60-hour CMA course is required. Qualified candidates will be reimbursed for the cost of the course and will receive a $500 bonus for completing the course.
The Clinic Patient Access Representative greets and assists patients in person and on the phone, schedules appointments by computer, takes messages, enters and verifies patient information. The Patient Access Representative will work as a member of a team to provide a high level of customer service to our patients. Other general office duties are also required. The Director of Senior Care Services directs, assesses, plans, implements and evaluates delivery of all senior care and dining services in resident-centered care and outstanding customer service. She/he provides administrative direction to the development and evaluation of all Senior Care (Independent Living, Assisted Living, Nursing Home) and Dining Services personnel and serves in the Nursing Home as the licensed administrator. The Office Coordinator is the “first face” visitors see when entering our senior care facilities. This position will be located at Landsmeer Ridge Retirement Community. S/he performs general office duties, and greets and assists families, visitors, and residents in person and on the phone. S/he maintains and manages the billing system for Landsmeer Ridge. The Radiologic Technologist performs a variety of Radiology duties in a busy imaging department. Candidate must be proficient in x-ray. He/She may have the opportunity to be cross-trained in one or more additional modalities including CT, MRI, and/or Mammography. Number of Hours: Part-time (approximately 16-24 hrs/wk.) The Emergency Medical Technician (EMT) provides patient care in the pre-hospital setting and during inter-facility transfers, according to Orange City Area Health System Ambulance protocols. The EMT will work as a team member with other EMTs and Paramedics. Number of Hours: Casual: 0-12 hours/week Variable – no regular schedule. Up to 12 hours/week, working one weekend every 8 weeks. The ambulance is staffed by on-call EMTs 24/7. The call schedule is flexible with other work/school schedules. The Prairie Ridge RN/LPN acts a team leader at Prairie Ridge Care Center. He/she administers med/treatments and supervises and directs care for approximately 24 – 48 residents. Assesses resident needs, provides input to resident’s plan of care, and provides and documents resident care as identified on the plan of care.
