February is Heart Month, and YOU have an opportunity to support the American Heart Association and bring awareness to the fight against heart disease by purchasing a T-shirt (or two or three…)!

Print this handy dandy order form and bring it with your payment (cash or check preferred but we can write down your debit/credit card info) to the Admissions desk at the Orange City, Hospers, or Paullina Clinics, or our Emergency Room desk in Orange City — by January 22 at 4pm. There are also order forms available in our Gift Garden gift shop, and in the clinics.
Pick-up dates are February 3 from 8am-12noon, and February 4 from 1-5pm in our Orange City campus front lobby.
All proceeds go directly to the American Heart Association.
