The Diabetes Care Coordinator is a Registered Nurse who demonstrates in-depth knowledge and clinical expertise in diabetes as well as diabetic patient education. He/she will manage diabetes care and delivery of diabetes self-management education for select patients and/or caregivers through referrals by nurses, doctors and self, which will include both inpatients and outpatients. The Diabetes Care Coordinator will also serve as a health coach and care coordinator to the diabetic population.
Hours: Full-time: 32 hours/week, with the potential for 40 hours/week.
Schedule: Monday – Friday day hours with some evening educational programs.
Educational Requirements: RN licensure. Bachelor’s Degree in Nursing with minimum of 3 years of professional nursing experience in a clinical setting preferred. Certified Diabetic Educator or willing to become certified.
Other: The applicant must have excellent clinical and interpersonal skills. He/she should possess strong computer skills and the ability to work with the electronic health record.
